چکیده

BACKGROUND The need for and consequence of sacrificing the buccal branch of the facial nerve during parotidectomy is unknown. We sought to determine the indication, frequency, and functional outcome of buccal branch sacrifice. METHODS We conducted a prospective study of all cases of parotidectomy at a tertiary referral center. RESULTS Of 100 consecutive cases of parotidectomy, the buccal branch was sacrificed in 23 cases. This subgroup was more likely to have anterior or deep lesions (p < .001), retrograde facial nerve dissection (p = .037), and immediate postoperative upper and lower facial weakness (p = .051 and .002, respectively). However, if the temporozygomatic and cervicomandibular branches were anatomically preserved, full facial (including buccal) function was restored. CONCLUSION Deep or anterior lesions may warrant sacrifice of the buccal branch for adequate access and excision. However, this does not result in long-term impairment of facial function. © 2016 Wiley Periodicals, Inc. Head Neck 38: 1821-1825, 2016.

Facial Nerve Monitoring During Parotidectomy: A Two-Center Retrospective Study

Introduction: We present a retrospective two-center study series and discussion of the current literature to assess the benefits of facial nerve monitoring during parotidectomy. Materials and Methods: From 2007 to 2012, 128 parotidectomies were performed in 125 patients. Of these, 47 procedures were performed without facial nerve monitoring (group 1) and 81 with facial nerve monitoring (group 2...

A cadaveric study of surgical landmarks for retrograde parotidectomy

PURPOSE Retrograde parotidectomy is employed in situations where tumors or scar tissue obscure the facial nerve trunk, making anterograde parotidectomy hazardous. Hence, the reliability of anatomical landmarks in retrograde parotidectomy is of equal practical importance. METHODS Distances from soft tissue and osseous landmarks to the corresponding peripheral branches of the facial nerve were ...

Promising Technique for Facial Nerve Reconstruction in Extended Parotidectomy

Introduction: Malignant tumors of the parotid gland account scarcely for 5% of all head and neck tumors. Most of these neoplasms have a high tendency for recurrence, local infiltration, perineural extension, and metastasis. Although uncommon, these malignant tumors require complex surgical treatment sometimes involving a total parotidectomy including a complete facial nerve resection. Severe fu...

Evaluation of Two Facial Nerve Landmarks Frequency in Parotidectomy

Background & Aim: Various landmarks are discussed to find the facial nerve during parotid surgery. The surgeon should use existing landmarks for a safe surgical use. To evaluate two new landmarks in parotid surgery, this study was done. Methods & Materials/Patients: This cross-sectional study was conducted on 43 patients with parotid masses, whom were referred to Alzahra and Kashani tertiary...
